Requirements for diabetic and hypertension patients :

• Hypertension patients will need to have blood pressure taken and medication list available prior to seeing the doctor/ nurse practitioner
• Diabetic patients will need to have blood pressure and blood sugar checked and medication list available prior to seeing the doctor/ nurse practitioner

Fees

•  We accept a wide range of insurance plans, including Medicare and Medicaid. We also offer flexible payment options for patients without insurance. Our goal is to make quality healthcare accessible and affordable for everyone.  
• Office visit prices are $85.00 for Self- pay and  $65.00 for a subsequent visit within 7 days for same Diagnosis 
• Co-Pay are accepted
• No payment for straight Medicaid/ Medicare 
• STD Package includes Visit with Doctor/ Nurse Practitioner, Labs and Medication. Price starts at $300.00
• Any prior visits with STI prices starts at $200.00     
• Any services listed under "Illnesses" will be required to pay $40.00 + office visit/subsequent pay prior to service.   
• Any services listed under "Requirements for diabetic and hypertension patients" will be required to pay $40.00 + office visit/subsequent pay prior to service.   
• Any services listed under "Other Services" will be required to pay $40.00 + office visit/subsequent pay prior to service.
